Forms

• Prior Absence Notification: Please pick up paper copy at Attendance Window.
• School Fees Form (click name) for school fees paid at the beginning of the school year or when enrolled after the beginning of the school year. Complete and Print Form. Drop off payment to the HVHS Bookkeeper.
• Parking Application: HVHS Parking Pass Application (click name), Complete and Print Form, Drop off payment (pricing information below) to the HVHS Bookkeeper.

Students must purchase a current decal and register their vehicle(s) through the bookkeeper’s office using the above form. A current decal must be displayed on the primary vehicle. Decals must be displayed on the lower left driver’s side of the rear windshield and will be issued for $40 each. Students purchasing decals after the first semester for valid purposes (just received license, etc.) are to purchase decals at $20 each. If a decal is lost or destroyed, students are expected to buy a replacement for $2.
